Role Summary

The Wellbeing Co-ordinator supports people aged 50+ in the towns and surrounding villages of Ashburton, Bovey Tracey and Buckfastleigh. Working with a range of NHS professionals, including GPs, Social Prescribers, Community Nursing Teams, Older People’s Mental Health Teams, Social Care and many other organisations, the Co-ordinator supports a person for 12-20 weeks to help improve their wellbeing. Managing a caseload of approximately 22 – 25 people, this challenging, but interesting  role, is very rewarding.

Working with partners, the role strengthens the voice of the person in communicating their needs and helps them to be active in managing their own wellbeing.

To support people when discharged from hospital for a period of 6 weeks if they need assistance, either through working directly with the patient or liaising with our Volunteer Team.

Helping to identify how to create community connections, whether through individual interests or hobbies, or through joining Moorland’s Moor Social events across the three towns.

Building trusted, person-centred relationships with health and wellbeing at its heart, the Co-ordinator helps people to connect, manage issues and engage with other professionals and organisations.

Tasks

  1. To carry out risk assessments, guided conversations and to listen to people about their needs, issues or plans
  2. To work within a strength-based approach, focusing on the person’s interests, aspirations, experience, and knowledge
  3. To offer encouragement and support to make community connections or access relevant resources of another organisation or charity
  4. To understand and develop community resources, network with colleagues across the NHS and Social Care and relevant organisations
  5. To facilitate Moor Social Events in the community, twice monthly
  6. To support, embrace and develop volunteer relationships
  7. To work professionally with Safeguarding Teams
  8. To record data in a timely and professional manner, sharing appropriately with colleagues or teams always respecting the client’s wishes and GDPR
  9. To be an advocate for an individual’s wishes within the locality, demonstrating an understanding of and maximising eligible DWP payments
  10. To conduct and produce evaluation of data and case studies which are meaningful to the client, the commissioners and MCC, reporting quarterly
  11. Any other tasks which are consistent with the role and job description
  12. To work flexibly when occasionally required, which may include working outside of contractual hours of work
